It is important to understand the payment policies outlined below:
If the answer to any question below is NO, you will not qualify for assistance:
1. Payment deposits will not be made in advance of approved treatment or surgery.
2. Payment will be made for the invoiced items related to the approved treatment or surgery only.
  3. Payment will be made up to the approved amount only.
  4. Payment must be made directly to the treating veterinarian. No reimbursements to pet owners will be made.
  5. Payment will not be made on an invoice showing a zero balance due. It will be assumed the pet owner was able to pay the invoice or make other arrangements.
  A check for the approved amount will be mailed to the treating veterinarian upon receipt of the required documentation listed on the Treating Veterinarian Information form.
